Your medical records have been transferred to secure storage at Medrecords.ca. and you may obtain your record from them by submitting a request either on their website, or by phone at +1 604 800 7079. This is now the only way to request your record. Please do not request a transfer of your file through this website, or by contacting the Executor of her estate. If you do, you will be re-directed to MedRecords. Her estate has generously agreed to pay on your behalf the usual fees charged by MedRecords for transfer of patient files, on one occasion only, provided that the request is made prior to December 1, 2021. Her estate reserves the right to change this policy without notice at any time.
If you requested your record prior to March 1, but your physician’s office has not yet received it, you will also need to resubmit your request to MedRecords, either on their website, or by phone at +1 604 800 7079. You will soon receive an automated phone or text message to inform you of this requirement. Your new family physician has also been advised of this requirement.
Any renewal medications, orders, referrals and requisitions (including for x-rays, scans, MRIs, ultrasounds, echocardiograms and other sorts of imaging) made by Dr. Trebell are no longer valid. This includes any prescriptions you may have in your possession but not yet filled, and any lab or x-ray requisitions that you may have, but not yet acted on. Please make arrangements for new prescriptions and tests with a new family physician.
If you are uncertain about the status of any pending test result, please discuss this with your new physician or walk-in clinic. Your new physician will be able to obtain information about the results from the appropriate facility with your consent.
If you have an appointment booked at the Breast Screen Centre, you will need to find a new family physician or attend a walk-in clinic before attending that appointment. You will need to give the Breast Screen Centre your new physician’s name when you check in. If you arrive at the Breast Screen Centre without having done this, they will not perform the investigation, but instead will reschedule your appointment.
